Alumni Records Request Form

University employees (faculty, staff and administrators) may use this form to request information about alumni. Your request will be reviewed and processed by Advancement Services. Please allow at least two weeks for your request to be processed. (Please note, if your department/division has an administrative assistant, you should check with them first, as they may also be able to access the information you are requesting.)

This form must be submitted electronically using the “Submit My Request” button at the bottom of the page. Please do not print this form and submit it in hard copy. (If you have difficulty with the form, please contact Mark Graves at mark.graves@uncg.edu.)
